How it works

1. Download and print the form.
2. Fill in your account holder, bank account and payment details.
3. Sign and date the form. Unsigned forms can't be processed.
4. Write your order number (if you have one) at the top of the form.
5. Mail the signed form to:

We'll process your payment once we receive your signed form. For your security, please don't send bank account numbers by regular email.
